This was a 31 years old Southern Chinese gentleman presented with Angle class III malocclusion, average mandibular plane angle, on a skeletal class III base.
He had a straight lateral profile, and his chin was mildly deviated to his left side.
There was premature contact at 22 which was in crossbite and caused mild mandibular forward displacement. Upper incisors were proclined and lower incisors were retroclined. Overjet and overbite were reduced in maximal intercuspal position. Lower dental midline shifted to the left side. All lower molars' crowns are tipped lingually. There were mild crowding on upper arch and moderate crowding on lower arch.
Accept skeletal discrepancy in sagittal and transverse dimension
Achieve class I molar, canine and incisor relationship
Correct dental midline discrepancy
Relieve crowding, alignment and leveling of the arches
Relieve mandibular displacement
Extraction of teeth 18, 28, 38, 48
Lower arch: expansion, rounding off the arch and IPR to relieve crowding. Lower anterior teeth lingual root torque to preserve labial periodontal support.
Upper arch: expansion, and IPR to relieve crowding. Tooth 22: staged movement: Intrusion to relieve occlusal interference. Then crown proclination to correct the anterior crossbite and extrusion to compensate for the relative intrusion during crown proclination.
