An 18-year-old woman, whose chief complaint was anterior upper teeth protrusion. She presented with a severe skeletal class II malocclusion and high mandibular plane angle, who refused to orthognathic surgery. There was a mild crowding in both arches and abnormal Bolton index in anterior teeth. She was treated with camouflage orthodontics of extraction of 14,24,34,45 using clear aligners. Total treatment time was 40 months. Both her occlusion and facial appearance were significantly improved by the upper and lower teeth retraction . Sagittal: Achieve canine and molar neutral relationship; Obtain normal overjet (within 1 mm)
Vertical: Obtain normal overbite (within 2mm ).
Transverse: Obtain upper midline consistancy with lower midline.
Others: Coordinate bolton index.
Sagittal: Retraction of upper and lower incisors; ( strong anchorage for maxilla (no mesial movement of molars in upper arch) and allow right mandibular molar to move mesially to obtain neutral relationship)
Vertical: Intrude upper incisors by 2-3mm, and intrude lower incisors by 2-3mm. Maintain or intrude the vertical position of molars;
Transverse: Move upper medline to right by 1mm and make lower midline consistant with the upper midline.
Others: Allow 33-43 stripping.
