Treatment plan 1. The treatment plan was : (1). to close the anterior open bite by means of a combination of retraction and extrusion of the upper incisors;(2). to close the anterior open bite by intrusion of posterior maxillary dentition; (3). to enable subsequent autorotation of the mandible;(4).to level and align the upper and lower dentitions; (5).to achieve Class I molar and canine relationships, as well as ideal overbite and overjet;
(6).to improve the facial profile and obtain natural lip competence without mentalis strain; (7).to expand the upper dental arch so as to improve the smiling8.to move the lower teeth on the right side to correct the middle line deviation through the extraction space.

Treatment Results: The patient was extremely happy with the treatment results and reported being comfortable while using the Invisalign System. After 33 months of therapy, treatment objectives set in the pre-treatment plan were achieved with good esthetic and good inter-cuspation of the teeth. The patient’s incisors were retracted and uprighted, improving her lips and profile esthetics with a harmonious straight-type profile. The anterior open bite had been completely closed; a normal overbite and overjet had been corrected and Class I molar and canine relationships had been established. In addition, the upper and lower dental midlines were coincident with the facial midline. Good root parallelism were achieved between extraction sites and adequate posterior anchorage were maintained.
The post radiographs show that aligners determined an intrusion of maxillary and mandibular molars that did not exceed 1 mm. A small amount of molar intrusion was achieved together with extraction and retroclination of the upper and lower incisors. The pre-post lateral radiograph did not show rotation of the mandible but mainly an extrusion of upper and lower incisors; a minor intrusion in the molar region was probably due to the minor intrusive effect of the aligners.
